چکیده

Background: To determine the role of Tiotropium as step up therapy for patients with uncontrolled asthma. Methods: In this case control study 130 patients, with diagnosis of bronchial asthma for at least 01 year and presently on step 4 treatment and still symptomatic, were included . Sixty five patients were randomly assigned to receive Tiotropium bromide dried inhaler added to their conventional step 4 treatment, whereas 65 patients were given placebo. Patients were asked about symptomatic improvement, using mini AQLQ. Peak Expiratory Flow Rate (PEFR) was checked eight weeks after wards to document response to therapy. Results: Mean age and duration of diagnosis of asthma was 38 years and 5.1 years, respectively in cases. Mean PEFR at admission among cases was 210 l/min (± 58.35) while it was 203 l/min (± 57.32) among controls. Frequency of nocturnal awakening were 2.98 days/ week among cases and 2.88 days/ week among controls. At week 8, frequency of nocturnal symptoms was found to be 1.6 days/week among patients on Tiotropium while it was 2.13 days /week among controls. Mean PEF was 387 l/min among Tiotropium group as compared to 305 l/min. There was statistically significant improvement in symptoms and PEFR of patients on Tiotropium as compared to controls. (p value 0.004). The differences remained statistically significant after adjusting for age and gender. Conclusion: Tiotropium daily as maintenance treatment through dried inhaler in addition to at least high-dose inhaled corticosteroids combined with long acting B2 agonists offers significant potential to improve airway patency in patients with severe persistent asthma who are still symptomatic and obstructed on maximal therapy.

Use of tiotropium bromide in an adolescent with adrenal suppression secondary to inhaled mometasone furoate

Introduction With emerging cases of adrenal suppression being reported in pediatric patients, finding appropriate steroid sparing agents is of clinical importance. Tiotropium bromide (TB), a long-acting anticholinergic, has recently been investigated in adults as a steroid-sparing agent in asthma step-up therapy. We report the first case of tiotropium bromide (TB) used as a step-up steroid spar...

Tiotropium bromide in asthma patients: an alternative to inhaled long-acting beta-agonists?

SUMMARY Asthma affects an estimated 5.4 million people in the UK. 1 Many patients remain symptomatic despite regular inhaled therapies. Given the burden of asthma and the heterogeneity of patients, the role of new therapeutic options requires evaluation. Tiotropium bromide has a well-established role in the management of chronic obstructive pulmonary disease (COPD). This is the first comprehens...

Positioning of Long-Acting Muscarinic Antagonists in the Management of Asthma

Despite a range of efficacious therapies for asthma, including inhaled corticosteroids (ICS) and long-acting β₂-agonists (LABA), a significant proportion of patients have poor asthma control and retain a risk of future worsening of their symptoms. Long-acting muscarinic antagonist (LAMA) bronchodilators offer a well-tolerated, efficacious, and cost-effective add-on to a patient's treatment. Of ...

Add-on Therapy for Symptomatic Asthma despite Long-Acting Beta-Agonists/Inhaled Corticosteroid

Asthma, remains symptomatic despite ongoing treatment with high doses of inhaled corticosteroids (ICS) in conjunction with long-acting beta-agonists (LABA), is classified as "severe" asthma. In the course of caring for those patients diagnosed with severe asthma, stepping up from ICS/LABA to more aggressive therapeutic measures would be justified, though several aspects have to be checked in ad...

Tiotropium bromide step-up therapy for adults with uncontrolled asthma.

BACKGROUND Long-acting beta-agonist (LABA) therapy improves symptoms in patients whose asthma is poorly controlled by an inhaled glucocorticoid alone. Alternative treatments for adults with uncontrolled asthma are needed. METHODS In a three-way, double-blind, triple-dummy crossover trial involving 210 patients with asthma, we evaluated the addition of tiotropium bromide (a long-acting anticho...
